Service spotlight: Patient Experience Team, whole system reports

The Patient Experience Team has a rich archive of comprehensive whole system reports, which include topics like continence, foot health, admission avoidance, safe discharge from the hospital and the exploration of support for informal carers.

At the Care Group, we are proud of the many services and activities we deliver, often in collaboration with partners across local healthcare. But how do we measure the effectiveness of our services and the work we do? Our Patient Experience Team is a well-established service that uses a variety of ways to gather the views of service users and carers who receive support from integrated community services in Tower Hamlets.

Established in 2017, the Patient Experience Team is responsible for capturing in-depth reviews and feedback of local people who receive health, social care and housing support. The team does this through a variety of ways, including real-life feedback, through home and group visits, as well as video or telephone calls. The team gathers this information and then makes recommendations for cross-organisational and system improvements. Often-times, this feedback is compiled into a “whole system review” report that is shared with the commissioner of the report in order to better understand what is working well and to identify opportunities for potential improvements. 

Some of the areas covered over the years include continence, foot health, admission avoidance, safe discharge from the hospital and the exploration of support for informal carers. We are in the process of publishing these archive reports and encourage you to read them via the links below. 

Shona Davies, Patient Experience Manager, said: “We are pleased to be sharing our whole system reports and we encourage people to read them to gain a better understanding of some of the excellent work and challenges within these health areas. 

“We are also in the process of carrying out a number of reviews, including P-RESET/RESET services as well as a whole-systems review on falls services within Tower Hamlets.”
