Day in the Life of a Care Group Colleague
Tower Hamlets GP Care Group formed in late 2013 to enable General Practices to be more involved in the local commissioning of health services in the borough. The GP Care Group is a federation, a system that allows a group of general practices to come together as an organisation to share responsibility for delivering quality services to its local population. GP Care Group enables all 36 practices in Tower Hamlets to speak with one voice, working collectively on innovative ventures to deliver primary and community services at scale, i.e borough-wide.
The GP Care Group is the lead manager in the “Alliance” between NHS Barts Health NHS Trust (the borough’s acute or hospital services provider), and East London Foundation Trust (ELFT), who provide a range of community, mental health and inpatient services. The Alliance is responsible for providing community health services in Tower Hamlets, taking responsibility for our GP Out of Hours service, single point of access, health advocacy, interpreting services and the patient experience team.
The GP Care Group is also one of six partners in Tower Hamlets Together, the collective through which the borough aims to achieve its ambition of improving the health and wellbeing of Tower hamlets residents through partners working together and developing a shared culture to provide person-centred, coordinated care to everyone who uses our services.
