Managing referrals

The School Health and Wellbeing Service (SHWS) has reviewed the current on-line referral form and process of managing referrals for Individual Health Care Plans (IHCPs) and Health Needs Assessments and has now revised the referral form and referral process in response to feedback and comments from users.

Referral Form

The new referral form makes it easier for colleagues to complete and retain a copy for future reference. Please do the following:

save it onto your computer or laptop
complete it
email it back to: thgpcg.schoolnurses@nhs.net
