The School Health and Wellbeing Service has reviewed the current online referral form and process of managing referrals for School Management plans (SMP) and Health Needs Assessments.
Once your child starts school in Tower Hamlets, the School Health team/school will inform you of how to access the Parent Portal to complete a health questionnaire about your child/children.
